A hospital in Lahore asks three suppliers for the cost of a 150 m³/day treatment plant. One quotation covers only a packaged biological unit. Another includes tanks, pumps and chlorination. The third includes drainage changes, standby equipment, sludge dewatering, installation, training and two years of operation. The three totals cannot be compared until every bidder prices the same boundary.
Hospital sewage and effluent treatment plant cost in Pakistan is not determined by bed count or flow alone. The price changes with the drains included, influent quality, peak loads, discharge or reuse objective, civil works, redundancy, power arrangement, sludge route, imported components, testing and operating responsibility.
This guide explains what a hospital STP or ETP quotation should include and how an owner can compare proposals. It does not publish a generic PKR range because no current public data set was found that compares complete, equivalent hospital wastewater projects in Pakistan. Repeating undocumented supplier ranges would hide the assumptions that actually control cost.
Why one price per bed or cubic metre is unreliable
Two hospitals with the same bed count can have very different wastewater. Outpatient activity, kitchens, laundries, dialysis, laboratories, cleaning practices, water conservation, occupancy, staff numbers and stormwater connections all affect flow and load. An operating hospital may also have old sewers, undocumented bypasses or tanks that are difficult to reuse.
A useful price requires at least five definitions:
- What enters the plant? Identify sewage-like wastewater, kitchen and laundry flows, specialist liquid streams, stormwater and excluded wastes.
- How much arrives? State average, maximum-day, peak-hour and minimum flow, plus batch releases.
- What must leave? Define the discharge or reuse route, applicable parameters and test method.
- What is physically included? Separate process equipment, civil works, electrical works, drainage changes, buildings and site reinstatement.
- Who operates it? State staffing, chemicals, testing, maintenance, spares and performance responsibility.
The hospital wastewater treatment planning guide explains how to map streams, measure flow and plan sampling before requesting prices.
The cost is a complete project, not a tank and equipment list
| Cost group | Items that may belong in it | Common omission to check |
|---|---|---|
| Investigation and design | Survey, drain mapping, flow study, sampling plan, laboratory analysis, process design, hydraulic profile, drawings and approvals support | A price based only on beds and an assumed BOD concentration |
| Civil and structural works | Excavation, dewatering, concrete tanks, foundations, waterproofing, buildings, access platforms, drainage and reinstatement | Rock, groundwater, shoring, restricted access, existing-utility protection and structural verification |
| Mechanical process equipment | Screens, pumps, blowers, mixers, aeration, clarifiers or membranes, filters, dosing, valves, pipework and lifting equipment | Duty and standby arrangement, material grades and equipment needed outside the packaged unit |
| Electrical and control | MCC, VFDs, cabling, earthing, level and flow instruments, alarms, automation, generator interface and power-quality protection | Incoming feeder, transformer or generator capacity, cable routes and automatic restart after an outage |
| Disinfection and polishing | Filtration, chlorination or UV, contact volume, residual or intensity monitoring and safe chemical systems | Guaranteed inlet quality, contact conditions, UV transmission, chemical demand and off-spec diversion |
| Sludge and residuals | Screenings, grease, waste sludge, storage, thickening, dewatering, testing, transport and final route | A biological plant that produces sludge but includes no method to remove it from site |
| Installation and commissioning | Freight, unloading, erection, testing, inoculation, process stabilization, operator training, manuals and performance test | Who pays while the plant is stabilizing or if influent conditions differ from the design basis |
| Operation and maintenance | Operators, supervision, electricity, chemicals, laboratory tests, calibration, preventive maintenance, membranes and spares | A low capital price with undefined annual operating duties |
Capacity should include flow and pollutant load
A quotation stating “150 m³/day” describes volume, not treatment duty. Ask for the design concentrations and daily loads for BOD, COD, TSS, oil and grease, nutrients and any site-specific parameter. Also request the peak-flow basis and equalization volume.
For each parameter:
Daily load in kg/day = flow in m³/day × concentration in mg/L ÷ 1,000.
Hypothetical teaching example: If measured average flow is 150 m³/day and representative BOD is 300 mg/L, the average BOD load is 45 kg/day. If another hospital also produces 150 m³/day but BOD is 500 mg/L, its load is 75 kg/day. Equal flow does not mean equal aeration, biomass, sludge or operating cost.
The proposal should state present flow, future design flow, pollutant-load ranges, peak conditions and the evidence used. See the worked wastewater-flow guide and BOD, COD and TSS guide for measurement and load calculations.
Influent quality and stream separation change the quotation
A hospital drain network can include ordinary sewage, kitchen grease, laundry water, laboratory liquids, dialysis-related flows, cleaning discharges and wastewater from waste-treatment equipment. Some streams may be compatible with the biological plant after assessment. Others may need prevention, separate collection or pretreatment.
Combining every drain without characterization can make the main plant larger while still failing to manage a concentrated chemical or pharmaceutical stream. Source control may reduce downstream cost, but it must be supported by labeled containers, staff procedures, collection responsibility and an approved final route.
The World Bank Group’s September 2026 guidance for health-care facilities identifies wastewater sources across clinical and support departments and recommends preventing hazardous solid and liquid waste from entering wastewater. It also recognizes that additional treatment may be necessary where active pharmaceutical ingredients or other hazardous constituents are present. A supplier should therefore list which hospital streams its guarantee covers and which it excludes.
Civil works can be a major difference between quotes
A packaged treatment unit is not a complete site. Even a containerized system may need an inlet chamber, screens, grease control, equalization, pumping, sludge storage, foundations, access, drainage, contact or treated-water storage and a safe chemical area.
Before bidders price civil work, issue:
- A topographic and utility survey with the proposed inlet and outlet levels.
- Available geotechnical information, groundwater observations and excavation restrictions.
- Tank volumes, operating levels, freeboard and structural design basis.
- Existing structures proposed for reuse, with condition and watertightness assessment.
- Access routes for excavation, cranes, concrete, equipment removal and future maintenance.
- Requirements for odour control, covers, ventilation, noise, safe access and confined-space work.
- Limits on hospital shutdowns and temporary bypass arrangements during construction.
A quote that says “civil works by client” should still provide dimensions, loads, openings, embedded items, waterproofing requirements and a clear interface schedule. Otherwise the owner cannot price the missing civil scope reliably.
Equipment quotations need duty, standby and material details
Count alone is not enough. Two quotations may both list two blowers, yet one assumes one duty and one standby while the other needs both units running at design load. The same question applies to pumps, mixers, dosing systems, membrane trains and disinfection equipment.
For every major item, request:
- Required duty point, normal operating point and efficiency.
- Number of duty, standby and installed spare units.
- Motor rating, absorbed power, control method and expected operating hours.
- Wetted materials, coating, corrosion allowance and environmental rating.
- Manufacturer, model, country of origin and approved equivalent policy.
- Consumables, recommended spares and local replacement lead time.
- Access needed to remove or overhaul the equipment.
- Warranty start, exclusions and support after commissioning.
Imported membranes, instruments or specialist equipment can add freight, customs, currency and lead-time exposure. The quote should state the currency basis, validity period, taxes, Incoterms where relevant, local freight and who carries exchange-rate risk.
Disinfection cost depends on a defined barrier
“Chlorination included” or “UV included” is not a design basis. A chlorination quotation should identify chemical form and strength, dose range, demand assumption, contact conditions, residual monitoring, safe storage and neutralization where needed. A UV quotation should identify design flow, required dose, UV transmission basis, lamp arrangement, intensity monitoring, cleaning and replacement assumptions.
Both systems depend on upstream treatment and solids control. Neither proves removal of every pharmaceutical, chemical or antimicrobial-resistance risk. The quotation should name the performance objective and the inlet conditions under which it applies.
Sludge handling belongs in the capital and operating budget
Biological treatment converts part of the incoming pollution into biomass. Screens, grease traps, chemical treatment and filters create additional residuals. If the quote ends at an aeration tank or clarifier, it leaves the hospital with an incomplete plant.
Hypothetical teaching example: If a process produces 60 kg/day of dry solids and waste sludge is withdrawn at 2 percent dry solids, the liquid sludge mass is 3,000 kg/day, roughly 3 m³/day when density is near water. Dewatering to 18 percent dry solids would produce about 333 kg/day of wet cake before allowing for capture losses and any added polymer. The equipment, storage and transport requirements are very different.
Ask bidders to state the assumed dry-solids production, withdrawal concentration, storage days, dewatering capacity, cake dryness, solids capture, polymer basis, drainage return and final handling responsibility. The screw press versus decanter guide and sludge drying-bed guide explain these choices.
Power cost should be calculated from equipment operation
Connected load is the maximum installed motor capacity, not monthly consumption. A useful operating-cost schedule lists each motor, absorbed power, duty units and expected run time.
Monthly energy in kWh = sum of absorbed kW × operating hours per day × days per month.
Hypothetical teaching example: Plant A averages 18 kW continuously and Plant B averages 12 kW continuously. At 24 hours per day and 365 days per year, the estimated annual energy is 157,680 kWh for Plant A and 105,120 kWh for Plant B. The difference is 52,560 kWh per year. Multiply those quantities by the hospital’s applicable effective electricity cost and evaluate demand charges, taxes and adjustments separately.
Do not freeze a generic rupees-per-kWh number into a long-term comparison. NEPRA publishes utility-specific tariff decisions and periodic adjustments, including a dedicated LESCO tariff record for Lahore. Use the hospital’s current tariff category and bills, then test more than one future tariff scenario.
The electrical quote should also cover sanctioned load review, transformer or feeder implications, backup generator load, automatic restart, harmonic or surge protection, cable routes, earthing, controls and alarms. A low-energy process is not reliable if the plant cannot restart safely after an outage.
Chemical and consumable cost needs a dose basis
A chemical line should state product, concentration, active strength, expected dose range, daily consumption, storage volume, dosing-pump duty, safety equipment and procurement responsibility.
Hypothetical teaching example: At 150 m³/day and a product dose of 50 mg/L, consumption is 7.5 kg/day. Over 365 days that is 2,737.5 kg before accounting for start-up, cleaning, wastage or changing wastewater. If the supplier quotes only “one dosing pump,” the owner cannot estimate chemical cost.
Apply the same logic to membrane cleaning chemicals, chlorine reagents, pH correction, nutrients, polymer, replacement lamps, filters and calibration solutions. Avoid a guaranteed fixed consumption unless the influent range and control conditions are defined.
Operation and maintenance should be priced as a service scope
Hospital wastewater treatment runs every day. A complete operating proposal should explain staffing by shift, supervision, sampling, laboratory testing, chemical preparation, housekeeping, preventive maintenance, calibration, sludge handling, reporting and emergency response.
| Operating item | Quantity basis | Evidence to request |
|---|---|---|
| Electricity | Absorbed kW × units operating × hours | Equipment schedule, control philosophy and measured commissioning baseline |
| Chemicals | Flow × dose, adjusted for product strength | Dose range, product specification, storage and trial or process basis |
| Lab testing | Parameters × locations × frequency | Sampling plan, methods, laboratory responsibility and reporting format |
| Staff | Roles × shifts × coverage days | Organization chart, qualifications, leave relief and emergency callout |
| Sludge and residuals | Wet quantity × collection frequency | Dry-solids basis, storage, tests, transporter and accepted final route |
| Spares and replacements | Preventive interval and expected service life | Two-year spares list, longer-term replacement schedule and lead times |
Separate guaranteed quantities from estimates. State whether O&M is included in the capital contract, offered as an optional service or retained by the hospital after training.
Compare quotations with one scope matrix
Issue one request for quotation and require bidders to complete the same response schedule. Do not force unlike technologies into identical equipment lists, but require each bidder to meet the same inlet, outlet, reliability, testing and interface requirements.
| Comparison line | Bidder must state | Owner should verify |
|---|---|---|
| Design basis | Flow, peaks, concentrations, loads, temperature and excluded streams | Values trace back to measured records and representative samples |
| Performance | Outlet parameters, averaging period, test method and guarantee conditions | Matches the actual discharge or reuse route and authority requirements |
| Process | Flow diagram, mass balance, hydraulic profile and control philosophy | No hidden bypass, undersized equalization or unsupported removal claim |
| Capital scope | Civil, mechanical, electrical, instruments, installation and site integration | Every exclusion has an owner-side price and responsible party |
| Reliability | Duty/standby equipment, storage, alarms, backup power and failure mode | Hospital can continue safely during maintenance and outages |
| Operating cost | Power, chemicals, staff, tests, sludge, spares and replacements | Calculations use comparable hours, tariffs and quantities |
| Delivery | Submittals, procurement, civil work, installation, start-up and test dates | Dependencies, shutdown windows and long-lead items are realistic |
| Commercial | Taxes, currency, validity, payment, warranty, security and variation procedure | Contract allocates exchange, quantity and performance risks clearly |
Public hospitals must use the rules that govern their procurement
A private hospital’s internal purchasing policy is not the same as public procurement law. A federal public-sector hospital should review the rules and standard documents applicable to its procuring agency. Provincial public hospitals should use the framework applicable in their province.
The official Federal Public Procurement Rules 2026 page requires appropriate and unambiguous evaluation criteria in the bidding documents. It also sets a process for examining an abnormally low bid in relation to scope, methodology, schedule, risks and responsibilities. The Punjab PPRA downloads page separately provides the Punjab Procurement Rules and procurement checklists. These sources support a practical lesson: define the scope and evaluation method before financial bids are compared.
This article is not a substitute for the hospital’s procurement, engineering or legal review. It also does not transfer federal rules to a provincial or private purchase.
Confirm the environmental and health-care waste basis
The Pakistan EPA rules and standards portal lists federal hospital-waste rules and NEQS documents. EPA Punjab’s PEQS page lists provincial standards for municipal and liquid industrial effluents and material related to biomedical waste. A hospital should verify the current notification, authority, discharge category, approval and monitoring duties that apply to its site.
Solid health-care waste, sharps, expired medicines and concentrated chemicals should not be priced as ordinary STP feed. The World Health Organization’s health-care waste management guidance is useful technical reading, but the project still needs the applicable Pakistani and provincial requirements.
Require commissioning and a performance test
Mechanical completion is not treatment performance. The contract should distinguish dry testing, wet testing, biological start-up, stabilization, operator training and the formal performance test.
Define:
- Required influent conditions and minimum operating period.
- Sampling locations, frequency, sample type and laboratory.
- Parameters, averaging method and acceptance criteria.
- Power, chemical and sludge data recorded during the test.
- Responsibility when influent is outside the agreed design range.
- Remedy, retest and handover procedure if the plant does not pass.
- Warranty start date and defects-response time.
Payment milestones can be tied to approved design, equipment delivery, installation, successful start-up, performance testing and handover rather than equipment arrival alone.
Quotation red flags for hospital owners
- Capacity is stated only as beds or m³/day, with no pollutant loads or peaks.
- The supplier guarantees every contaminant without defining tests or inlet conditions.
- Civil, electrical, drainage, installation or sludge work is excluded without interface drawings.
- All laboratory and clinical drains are assumed compatible without an inventory.
- Connected motor kW is presented as expected energy consumption.
- Chemical consumption has no dose, strength or wastewater basis.
- There is no off-spec holding, alarm response or plan for a power failure.
- Operation is described as automatic, but no staffing and maintenance schedule is provided.
- The price excludes taxes, freight, customs or currency exposure without stating them clearly.
- The offer is unusually low because essential scope is shifted to the owner.

Frequently asked questions
What is the price of a hospital STP in Pakistan?
There is no defensible universal price without a defined flow and load, drain boundary, outlet target, process, civil scope, electrical scope, redundancy and operating responsibility. Ask for a site-specific quotation against one common design brief.
Is an ETP always more expensive than an STP?
No. STP and ETP are labels, not fixed price categories. Cost depends on the actual streams and treatment duties. Source control or pretreatment can sometimes avoid sending an incompatible concentrated stream through a larger combined plant.
Can a containerized plant avoid civil cost?
It may reduce some on-site construction, but foundations, drainage connections, equalization, pumping, sludge storage, access, treated-water arrangements and electrical integration may still be required.
Should the lowest quotation be selected?
First determine whether each bid is technically responsive and covers the same boundary. Compare evaluated scope, exclusions, performance, reliability, operating quantities and contract risk. Public procuring agencies must also follow their applicable procurement rules.
How many years of operating cost should be compared?
Use a period that captures meaningful replacements and the hospital’s procurement policy. State assumptions for electricity, chemicals, labour, testing, sludge, escalation, discount rate and replacement timing so the comparison can be audited.
Does Aquenvia provide a free laboratory investigation?
No. Aquenvia can provide a free initial project discussion and help define the site-assessment, records and sampling scope needed for a quotation. Laboratory analysis, surveys and detailed engineering are separate project inputs.
Turn quotations into an owner decision
The best hospital wastewater quotation is not the longest equipment list or the lowest unexplained total. It is the proposal that makes the design basis, complete scope, performance conditions, interfaces, operating quantities and owner responsibilities visible.
Review Aquenvia’s hospital wastewater treatment service for assessment and project-delivery scope. Hospital owners, consultants and procurement teams can request a free initial site-assessment discussion or call 0325 7588341. The discussion does not include free laboratory analysis or a guaranteed compliance outcome.
